各位同學大家好，
為了能更多了解目前校園中導師在輔導學生上的狀況是如何，有哪些需要加強或調整之處、並票選最佳關懷教師，學務處製作了一個「導師問卷」。請各位同學花一點時間填寫(不到五分鐘)。問卷的位置在各位的校務資訊系統https://www.ccxp.nthu.edu.tw/ccxp/INQUIRE/ ，<導師聯繫資料>目錄下其中有一個<導師問卷>的選項，歡迎你填寫。
這份問卷不會有各位同學的個人資料或導師資料，希望各位同學可以放心作答、以提供我們和系所主管參考。填答時間從2026/05/15起至2026/06/07日午夜12點為止。感謝各位同學所提供的意見，作為我們未來導師輔導工作的參考。

Hello everyone,
In order to learn more about the current situation of advisors on the campus, and how strengthen or adjust this system, the Student Affairs Office has produced a questionnaire in this semester. Please take a moment to fill out (less than five minutes). The location of the questionnaire is in your school information system.

In the "Advisor/Professor contact information”, there is an option “Questionnaire on Advisory Relationship in National Tsing Hua University”.
About the Questionnaire, there will be no personal data or advisor's information for you to input. We only provide the results to Chairman in your department. We hope you could fill up this form and give us your feedback freely. The time for answering is from May. 15, 2026 to June. 7, 2026 at 11:59pm. Thank you for your comments and suggestions as a reference for our future advisor work.
We will also provide gifts for you! We will draw a lottery after the closing date June. 7, 2026 and notify the winners actively. Gifts are Logitech Pebble2, NTHU bag, NTHU Jacket, NTHU Picnic mat, NTHU Stainless Steel Multi-tool Card, and Key Chain.
